 understand that Eyelash Extensions are an optional, cosmetic service that I have requested. Individual response to the products used may differ. I will not hold the practitioners responsible for any ill effects that may result from this optional cosmetic service. If after any Eyelash Extension appointment, I feel burning, itching, any discomfort to the eyes or lids, or discover swelling of the eyelids, I understand that it is my responsibility to seek medical help at my own expense.  

Precise aftercare instructions will be given at the end of the service visit. Following these instructions will ensure the best, most durable results for semi-permanent eyelash extensions. I understand that I should not get my Eyelash Extensions wet within the first 12 hours of application or touch-up. I have been advised that using mascara on a regular basis can shorten the length of time my extensions remain in place. I have also been advised to never use waterproof mascara or waterproof eyeliner on or near my extensions. Even with strict adherence to these care instructions, it is impossible for the practitioner to guarantee exactly how long eyelash extensions will last. I understand that there are many variables that will influence how long my eyelash extensions remain in place. 

 I acknowledge that I should not rub my eyes or pull on my lashes or breakage and lash loss could result. If I am interested in keeping them looking their best, I understand that regular maintenance/enhancement appointments must be made to replace eyelashes that fall out due to wear and tear or their normal life cycle.
